This session highlights how Advancing Quality – Premier’s model of improving patient care while safely reducing costs with HQID – has been successfully implemented on the international stage. Launched in the United Kingdom's National Health Service North West, Premier’s partnership has worked to align positive financial incentives with improvements in patient care. Presenters will detail how this success was achieved by implementing evidence-based care practices in five clinical prevention areas along with data collection and performance improvement services. Participants will gain perspective on how to successfully improve patient care while safely reducing costs in a very different healthcare environment.

Net Revenue Advisor: Harnessing hospital net revenue analytics (BW-D)

This session highlights Premier’s Net Revenue Advisor – a new tool developed to support members’ efforts to identify revenue cycle issues and minimize bad debt. Participants will understand how the tool provides insight into variations in net revenue, as well as the benefits of calculating net revenue at the patient account level. Net Revenue Advisor quickly delivers an analysis of prior periods’ impact on current month’s net revenue and bad debt. Enabling quick comparisons to historical reimbursement run rates allows members to proactively identify and address potential revenue cycle problems.

Healthcare providers are becoming more aware that the period of VTE risk in patients extends beyond the hospital stay. This session will highlight the results of a study that evaluated VTE prophylaxis patterns for U.S. patients across the continuum of care, from the hospital setting through the physician office visits. Patients in the analysis were evaluated on drug utilization, diagnoses and clinical practice patterns during and after hospitalization.

This session will highlight Summa's efforts to reduce the incidence of ventilator-associated pneumonia (VAP) and describe how they have partnered with suppliers to improve clinical outcomes. The project demonstrated significant improvements resulting in a decrease in VAP rates to zero and capturing annual savings of $724,000. The team from Summa will describe their "a-ha moment" when they realized that the supplier could play a crucial role in helping them achieve the organization's clinical objectives. The session will also describe Premier's QUEST Comparative Innovation Program (CIP) and Summa's success in supporting this initiative. CIP tests the effectiveness of new healthcare products and technologies through the QUEST program. Participants will understand how to apply CIP findings to their own clinical improvement initiatives.

Through Premier’s Advancing Quality program, hospitals in the United Kingdom’s National Health Service North West are translating the success of the CMS/Premier Hospital Quality Incentive Demonstration project to their organizations. Beginning in January 2008, Wirral University Teaching Hospital NHS Foundation Trust implemented an active case management model by developing a process to identify patients using prospective, real-time data collection to enhance care planning. In this session, presenters will describe the process for educating staff at all levels to create superior clinical engagement, enthusiasm and ownership. Specific data for heart failure and pneumonia will also be presented and all tools that have been developed will be shared.

Modeled after IHI’s “Model for Improvement,” a proven breakthrough system, participants work collaboratively with peers from other leading healthcare organizations committed to accelerating breakthrough improvements. More than 250 teams have taken part in Premier’s BTS program, capturing cumulative savings of more than $150 million – a return on investment of 35:1 based on an average team savings of $750,000. Premier’s Collaborative BTS program and methodology will be reviewed as well as the methods and strategies used by previous participants to achieve savings and clinical and quality improvements.

Through Premier’s Advancing Quality program, hospitals in the United Kingdom’s National Health Service North West are translating the success of the CMS/Premier Hospital Quality Incentive Demonstration project to their organizations. In this session, participants will understand the development of Aintree’s heart failure service, which is led by cardiac nurse practitioners and over time has demonstrated a 50 percent reduction in hospital readmissions. Presenters will describe the program’s data-driven approach to assessing and managing patients with heart failure while promoting the highest standard of quality care. Data presented will include an investigation of wait times and treatment rates, as well as clinical audit findings and early evidence supporting the effectiveness of the service – including reduction of hospital readmissions and improved quality of life for patients. Results and overall feedback from patient satisfaction surveys will also be shared.
