Request for session proposals: Guidelines for submission
Thank you for your interest in presenting at the 2009 Breakthroughs Conference. Please review the following information and instructions carefully before submitting your proposal. The timeline for selection is posted below.
The information below provides a detailed explanation of the requirements to submit a proposal. Please have the following information available prior to clicking on the "submit proposal" link at the bottom of this page. If you wish to submit more than one proposal (but no more than two,) they must be submitted separately.

Primary contact information
One person is required to be the primary contact for each submission. This person is not required to be a speaker, but must be willing to coordinate the participation of and forward all communications to each speaker.
Please provide the following demographic and background information for the primary contact. You will be asked to provide the presenter names and information later in the process.

Proposals which do not include all of this information will be deemed incomplete and will not be considered for inclusion in the program.
Proposed presentation title
Please note that while Premier may change the final session titles for marketing purposes, we encourage you to use a brief but compelling, descriptive title. Titles may not exceed 10 words.
Session categories
Each session should illuminate an innovative process improvement or best practices case study, or provide them with more in-depth knowledge about familiar subjects. You will be asked to indicate the topic area which BEST describes your proposed session:

- Continuum of care (non-acute, alternate site)
- C-Suite
- Foodservice
- Leadership
- Management strategies
- Managing drug expense
- Operations

- Supply chain improvement

Proposed session description
Please create a succinct, one-paragraph description of your session. The description should focus on the features and benefits of attending, as well as the session's ultimate value to conference attendees. Sample descriptions are available for your review.
Along with your objectives and knowledge transfer goals, reviewers will use this information when determining the final session choices. If your session is chosen, portions of this description may be used in the marketing materials. SAMPLE

If known, please identify the Premier staff person with whom you work most frequently. If you are a Premier staff submitting on behalf of a member, enter your own name.
Learning objectives
Please enter three learning objectives for your session. Objectives should answer the question “As a result of attending this session, participants will be able to…” SAMPLE
Knowledge transfer goals
Please list 3-5 items that summarize each of the following. Due to formatting requirement, please do not insert bullets or numbers. SAMPLE

Skill level of audience
You will be asked to identify the skill level of your intended audience. Should they be familiar with your topic at a beginner, intermediate or advanced level?
Continuing education credits
Premier plans to apply for continuing education credit for the disciplines listed below. Be prepared to identify the appropriate categories for which you believe your session would qualify. You may also identify continuing education credits not listed here.

If one of the speakers is also the primary contact, you must re-enter this information.
Speaker biography
Speaker biographies are crucial to the planning process. They are required for CEU credit application purposes, for promotional materials and for introductions on site. Please include 4-6 sentences that reflect the speakers' current position and healthcare experience in the presentation subject.
Biographies must be included as part of the RFP submission process. Because of their importance, proposals not including speaker biographies cannot be considered. SAMPLE
Speaker skill level
Please let us know the skill levels of each speaker, as well as if the speaker has presented at any past Breakthroughs meetings. Please be prepared to enter the title of the past presentation and the session or track.
Evaluation criteria
Proposals will be evaluated using the following criteria:
- Relevance of topic to audience
- Timeliness of topic
- Expertise/experience of speaker(s)
- Presentation innovation and originality
- Partnership with Premier
- Ability to replicate
- Depth of information to be provided
- Potential for audience participation
Combined sessions
Occasionally, case study proposals focusing on similar topics are submitted by different organizations. When this occurs, submitters will likely be asked to work together to combine their case studies into one panel session. You will be notified in advance if this occurs and asked to select only one presenter from your organization to participate in the session.
