The Breast Health Services initiative serves low-income and minority populations in Boston and surrounding areas helping breast cancer patients in the hospital’s multi-cultural community access care more effectively and efficiently. Through outreach and care redesign like patient navigation, Boston Medical Center has increased access to breast cancer screening and treatment for underserved women and has reduced barriers to care for populations in need.

Adopt-A-Mom, Guilford County Coalition on Infant Mortality

Greensboro, North Carolina

Adopt-A-Mom coordinates prenatal care for low to medium-risk pregnant women who are Medicaid ineligible and lack private health insurance to obtain prenatal care. More than 80 volunteers work with seven local obstetricians who provide prenatal care at a flat fee of $400 per patient (paid by the Coalition on Infant Mortality).

Child Asthma Link Line

Philadelphia, Pennsylvania

Child Asthma Link Line coordinates the care of children with asthma living in medically underserved communities in North and West Philadelphia. The program seeks to understand the complexity of families impacted by asthma and link these families to a holistic range of services for improving health and quality of life.

Childhood Asthma Initiative – Children’s Health Fund

New York, New York

This initiative provides asthma services to homeless and poor children in New York City and Washington, DC. Since it was established in 1997, the Childhood Asthma Initiative has significantly reduced asthma severity and preventable hospital use among these medically underserved populations. The initiative’s approach includes significant community education.

South Bay Asthma Advocacy Program – Paradise Valley Hospital

National City, California

Paradise Valley Hospital established the South Bay Asthma Advocacy Program in 2002 to address the nation’s leading childhood chronic disease in its community. In addition to medication management components, this disease management program provides in-home education and environmental assessments by licensed respiratory care practitioners.

WINGS (Women Involved in Nurturing, Giving, Sharing)

San Antonio, Texas

WINGS was founded in 1999 by two breast cancer survivors to provide breast cancer treatment to uninsured women in South and Central Texas. The program offers breast cancer treatment, including surgery, chemotherapy, radiation, home care and medications, at no charge to the patient.

Birth Companions Program of Johns Hopkins University School of Nursing

Baltimore, Maryland

Birth Companions assists underserved pregnant women in navigating prenatal care and community resources to improve health outcomes. Since 1999, Johns Hopkins University nursing student Birth Companions have provided prenatal care advocacy, labor support and birth attendance assistance to at-risk mothers.

FREE – Foundation for Rehabilitation Equipment and Endowment

Roanoke, Virginia

FREE serves adults with physical disabilities who are unable to acquire equipment to gain greater independence. This volunteer-based organization’s mission is to provide mobility-related equipment to adults when no other public or private resources are available. Gently-used equipment (walkers, wheelchairs, hospital beds, power chairs, etc.) is moved through an intensive sanitation, inspection and repair process.

Health Connections – Youth & Family Services, Inc.

Rapid City, South Dakota

Health Connections, which began in 1992, is a health-focused case management program that assists high-risk underserved girls aged five to 15 gain access to existing community healthcare services. The program also assists the girls and their families in learning how to establish a primary care provider and access preventive and urgent care services.

Natural Detoxification Program – C.O.R.E. Center

Muscle Shoals, Alabama

This program is an outreach health prevention/lifestyle modification program which began in 2002 as a joint venture between Alabama Cooperative Extension System and The Shoals Natural Healthcare Center. The program was launched to teach detoxification, healthy food selection, food preparation and lifestyle changes to residents in three Northwest Alabama counties.

Steps to Success at Terrebonne General Medical Center

Houma, Louisiana

Steps to Success is a community-based parent education and prevention program dedicated to improving community health through parent support groups, parenting classes, teen pregnancy prevention programs, life skills education, community outreach and hotline services. The program was established in 1999 and targets teen mothers and the provision of prevention services to individuals age eight and older.

Safety Net Project – ViaHealth

The Safety Net Project was established to foster a seamless, coordinated healthcare delivery system that links patients directly to their personal physician and with additional community services across disciplines from any “point of entry.” Improved access for this program is defined as same-day access to your primary care provider; it is based on the advanced access model from the Institute for Healthcare Improvement.

Working Families Initiative of Bon Secours of Maryland Foundation

The Working Families Initiative was established in 2002 to improve access to healthcare for the people of West Baltimore by helping them obtain quality employment with health insurance benefits, requiring and providing referrals for infant and child immunizations and well-baby check-ups, and achieving an economically self-sufficient wage.
