This initiative represents a systematic building of a healthy community by setting long-term health improvement goals owned by both the community and its health systems. Five non-profit health systems and 50 community groups and organizations joined to create a new standard of health in their community. Funding of $800,000 comes from participating organizations for initiatives in reducing domestic violence and child abuse, adolescent health risk, breast cancer, and maternal and infant health over two years.
Outcomes:
- 19 percent increase in domestic violence victims who have a safety access plan
- Domestic violence victim referrals from healthcare providers have increased 134 percent; child abuse referrals have increased 12 percent through increase in trained health providers
- An 80 percent increase in transportation availability for domestic violence victims to travel to and from shelter and health facilities
- Adolescent health risk program has reduced school suspensions and conduct referrals by 67 percent
- More than 1,000 women have reduced smoking during pregnancy and 360 have actually quit smoking
- Breast cancer screening and promotion activities have impacted thousands of women in the community over the last two year
