Hospital Survey on Patient Safety Culture
The Agency for Healthcare Research and Quality (AHRQ) in partnership with Premier Inc., the Department of Defense and the American Hospital Association announce a new survey toolkit to assess hospital employee attitudes about safety. This survey tool can be used to assess the safety culture of a hospital as a whole, or specific units within hospitals, as well as to track changes in patient safety over time and evaluate the impact of patient safety interventions. This toolkit includes:

Survey instrument;
- Detailed user's guide for a paper or web-based survey;
- Methods for data collection and analysis;
- Customizable templates for reporting results;
- References; and
- Results of a pilot study.
