Premier developed this customized Excel™ data tool for easy entry and rapid display of the results from the surveys to provide immediate internal review and feedback. Important: For the file to work properly:

  1. Save it to your computer first. Right click on link below and select save as. (do not open directly from web site).
  2. This tool uses Excel macros. Before opening the file, be sure to set Excel's macro security level to "medium." From Excel's menu (without the file open) select Tools > Macro > Security. On the "security level" tab click "medium." Then open the tool and enable macros when prompted.

NOTE: Premier is unable to provide technical support for the use of this tool. If you have any comments or suggestions for enhancements of this tool, please send an email to safety_culture@premierinc.com.
