Hospital Survey on Patient Safety Culture – June 2008
Data submission deadline for comparative database

The AHRQ database serves as a central repository for hospitals to report their results from the AHRQ Hospital Survey on Patient Safety Culture. Participating hospitals are able to measure patient safety culture in their institutions and compare results with other sites. The deadline to submit data for this year is June 30, 2008

Hospital Survey on Patient Safety Culture-
Updated 2008 comparative database report

The 2008 report provides results from nearly 400 U.S. hospitals that can be used as benchmarks in measuring a culture of safety. The report consists of a narrative description of the findings and two appendixes, presenting data by hospital characteristics and respondent characteristics.

Based on data from 382 U.S. hospitals, the prior 2007 Hospital Survey on Patient Safety Culture Comparative Database Report provided initial results that hospitals were able to use as benchmarks in establishing a culture of safety.

AHRQ Benchmarking database initiative

In 2004, the Agency for Healthcare Research and Quality (AHRQ) released the Hospital Survey on Patient Safety Culture, a tool to help hospitals evaluate how well they had established a culture of safety in their institutions. A benchmarking database was also needed so hospitals and units in hospitals could determine how well they were doing in establishing a culture of safety in comparison to other, similar hospitals or hospital units.

The Hospital Survey on Patient Safety Culture: 2007 Comparative Database Report meets that need. Based on data provided voluntarily by 382 U.S. hospitals, the Report provides an initial set of results that hospitals can use as benchmarks in establishing a culture of safety in comparison to other, similar hospitals or hospital units.

Hospitals can use the results in the 2007 report as one basis for comparison in their efforts to establish, improve, and maintain a culture of patient safety in their institutions.

The body of the report presents statistics (averages, standard deviations, minimum and maximum scores and percentiles) on the patient safety culture areas or composites assessed in the survey as well as the survey’s items. Appendixes A and B present breakouts of the data by hospital characteristics (bed size, teaching status, ownership and control, region) and respondent characteristics (hospital work area/unit, staff position, interaction with patients).

Another round of voluntary data collection is planned to update the database with results from additional hospitals and evidence of change in the initial hospital set. Hospital Survey on Patient Safety Culture:
2007 Small or critical access hospital benchmarking
tool for AHRQ hospital survey

This tool was developed for small and critical access hospitals to use for benchmarking and comparison of their results from the AHRQ Hospital Survey on Patient Safety Culture. However, it can be used by used by any size hospital as it allows the addition of any benchmarking data.

The tool is from Statis Health and the Oklahoma Foundation for Medical Quality as the Hospital Interventions Quality Improvement Organization Support Center (HIQIOSC) and adapted with permission for a benchmarking tool developed by the Nebraska Center for Rural Health Research.
