The toolkit below includes the survey, detailed user's guide for administration of paper or web-based methods, data collection and analysis, customizable templates for reporting results, references, and results of the pilot study. Please note: The Spanish version of the data collection form has been replaced with the 2009 version. The Info and Dimensions file may be useful for the survey administrator explaining the changes made and should not be distributed with the actual survey forms.
