USP-Chapter 797 Pharmaceutical Compounding – Sterile Preparations, 2008
The U.S. Pharmacopeia (USP) released the final changes to General Chapter Pharmaceutical Compounding – SterilePreparations <797> and replaced standards and conditions for sterile compounding over the current version of <797>. The full chapter was posted as a Revision Bulletin on USP’s Web site on December 3, 2007. These revisions became official on June 1, 2008, and are included in USP 31-NF 36 and the second edition of the Pharmacists’ Pharmacopeia. The revised standards were published online to give the compounding community time to implement changes before the official date. The new standard became effective June 1, 2008.

This document presents guidelines for developing institutional programs to enhance antimicrobial stewardship, an activity that includes appropriate selection, dosing, route, and duration of antimicrobial therapy. The appropriate use of antimicrobials is an essential part of patient safety and deserves careful oversight and guidance. These guidelines focus on the development of effective hospital-based stewardship programs and do not include specific outpatient recommendations.
