Premier submitted comments to the Centers for Medicare & Medicaid Services (CMS) on the proposed Direct Contracting – Geographic model, which is part of the Primary Cares Initiative that CMS announced in April 2019. The Geographic option would assign a patient population of at least 75,000 Medicare fee-for-service beneficiaries within a targeted region to an entity that is willing to accept 100 percent up- and down-side risk for spending and quality outcomes. Premier’s comments in response to CMS’ request for information (RFI) focused on these key themes:
- Provider-led applicants should be given priority within the model. Providers have the ability to directly implement care delivery transformation and maintain a deep reservoir of pre-existing relationships with community-based organizations within their regions. These capabilities are necessary to support community-based care, address the social determinants of health, and support population health.
- CMS should establish rules for model overlap that provide protections for entities responsible for total cost of care. Alternative payment models currently responsible for total cost of care may by materially financially harmed if not provided historical precedence.
- Additional guidance on payment, included services and beneficiary alignment is needed.
