Henry Ford Allegiance Health (Jackson, MI) had experienced an increase in multiple admissions to the Emergency Department for reasons related to behavioral health. Seeing this, along with outdated programs, unclear roles and responsibilities, and significant disruptions in care, Henry Ford Allegiance Health - Behavioral Health knew that dramatic steps would need to occur to address these issues and the needs of the community and the hospital.
In 2015, Henry Ford partnered with Premier's Continuum of Care offerings, Labor Consulting Services, and Population Health Consulting Services to assess the current state of the behavioral health unit and create measurable objectives. From there, the health system began its journey to provide a cross-continuum care management program in order to align its behavioral health services with a care coordination process that span the continuum of behavioral health services and effectively employ community resources.
Since embarking on its journey, Henry Ford has seen streamlined points of access across the care continuum along with decreased wait times in the ED. They saw an uptake on contribution margin, reduced opioid overdoses and readmissions, and realized $430,000 in consolidated savings.
