The Medicare payment system, which sets the overarching incentives in health care, is a vital resource for people and providers. But is has also created care silos, a focus on sickness-based care and competition among health-care providers. To overcome this, new value-based models–such as value-based purchasing, bundled payment and shared savings–are being designed. These models have the potential to incent what health care needs – collaboration among physicians, hospitals, payers, corporations, even people.
